Gynecomastia can affect one or both sides and may involve firm glandular tissue, fat, skin excess or nipple projection. A chest that looks enlarged is not automatically treated in the same way for every patient.

A visible concern does not always have a single cause. The first consultation should identify the underlying anatomy, tissue characteristics, previous treatment and the outcome the patient actually expects.

- Direct gland excisionRemoves glandular tissue through an incision, often at the areolar border.
- Combined liposuction and gland excisionCommonly considered when both fat and gland are present.

Candidates usually have a stable chest appearance, stable weight and realistic expectations about scars, symmetry and recovery. New, painful or unilateral enlargement may require medical evaluation before cosmetic surgery.

Compression garments and activity restriction are common. The timing of drain removal, wound review, showering, exercise and flying must be decided by the surgeon. A patient should not leave Korea before the recommended early assessment.

Surgery can flatten and reshape the chest but cannot guarantee perfect symmetry, invisible scars or a bodybuilder contour. Excessive gland removal can create a crater deformity, while inadequate removal may leave residual fullness.
