Under-eye fat repositioning is not a universal treatment for every dark circle. Examination should assess the eyelid, cheek support, skin, pigmentation, dryness and risk of lower-lid malposition.

A visible concern does not always have a single cause. The first consultation should identify the underlying anatomy, tissue characteristics, previous treatment and the outcome the patient actually expects.

Candidates should have a clearly defined anatomical cause and understand that bags, hollowing, pigment and wrinkles may require different treatments. Dry eye, thyroid eye disease and lower-lid laxity require specific evaluation.

Surgery may improve bags and the transition between the eyelid and cheek, but it may not remove pigment or eliminate every line. Excessive fat removal can create hollowing. Male eyelids often require conservative design to preserve identity and expression.
